Update Your Contact and Pharmacy Information

The integration of electronic medical records makes it imperative that we have your current information at all times.  Please contact our office to update any personal information including a change in the pharmacy that you use. 

OHIP Coverage - Updating Your Health Card

Please ensure that your OHIP coverage is kept up-to-date at all times.  If your OHIP card is expired or due to expire soon, please visit the Ministry of Health website for information on how to renew your OHIP card. The Ministry of Health will generally send you a reminder notice approximately two months before your OHIP card expires.
