How can I ask my doctor medical questions or make medical requests?

For routine medical concerns please call the office to book an appointment.

** Please make every attempt to call and book an appointment, or use the after-hours clinics available in Angus, Beeton, Tottenham or Alliston during the week.

Dr. Seward will review all test results. If there is a concern with your test results, Dr. Seward will contact you to discuss treatment or staff will call you to book a follow-up appointment.

For urgent /critical situations outside of regular office hours or after-hours clinic hours - please proceed to the emergency department at your local hospital.

Medical Records

How can I request a copy of my medical records?

Please ask your new family doctor to fax a signed Chart Transfer Request to our office. A fee will apply.

For specialist referrals:

Dr. Seward will send the required medical reports to your specialist. If you are required to pick-up a copy of medical images/files, office staff will advise you of this and provide you with further information.
